- Printing Instructions (tested on Windows XP,7 with Internet Explorer)
An image of the form will appear above. This may take some time.
The form is complete when the last line finishes with "acceptance of this condition".
Wait for the image to complete and then right-click
on it and choose "save picture as ..." or similar wording from the menu.
The desktop is a convenient place to save it.
Later, when you are ready to print it, find the image file where you saved it
and right click on it and choose "print" from the menu. The original form is printed on A4-size paper.
If your printer is not set appropriately then use Printing
Preferences to select A4 paper size and possibly portrait mode before printing.
In some cases you may need to click a page layout tab to do this.
With an Operating System other than Windows, e.g. Linux, you may need to display the
the image in a viewer before you can choose to print it.
